[CQT-447] The compiler can read and process parameterized measure instructions

• Restructured the general decomposer to accept instructions
• Refactored the Replacer to decomposer/gate_replacer
• Refactored check_gate_replacement to check_gate_decomposition
• Added the MeasureDecomposer (and tests)
• Updated the Writer to accept parameterized measure instructions (and added tests)
• Updated CqasmV1Exporter to accept parameterized measure instructions (and added tests)
• Added entry to the Changelog

Still to be done:

• Update the User documentation
• Upgrade LibQasmParser to parse parameterized measure instructions.
