Prevent redundant comments across diffs for the same revision

Currently, a diff is reviewed in isolation. If a new version is submitted without addressing previous comments, there is a risk of generating redundant comments that convey the same meaning in different words. We should filter these out to prevent unnecessary distractions and keep the review process focused.
