What was confusing	The medical certificate was incomplete as her healthcare proxy they failed to notify me they to save me and said they were adjusting my mother's medication and they sent her home with pneumonia but how can they evaluate a dementia patient who has dementia in South Shore hospital is very aware of this we got the proxy done there because she couldn't make men up a decisions back in 2018 and the doctors knew about it the nurses knew about it I was always up there but the judge acted like it didn't exist when it was an affidavits from the opposing side South Shore Elder services she wrote I was her invoked healthcare proxy the doctors wrote that and they lied and said that the family and a little bit of family that she has is in agreement that was an absolute lie my concern is their case was built online they switched attorneys for my mother without even notifying me how can an attorney jump ship and leave my mother a couple weeks before her court date and why did not want attorney for my mother file any type of motion besides for funds how could they be her attorney and not meet with her this supposed to fight for my mother but they were just fighting for money I was a son interested party and healthcare proxy who hired an attorney who I already had for the housing case which my lawyer told me I was supposed to be in Canton on July 15th 2024 and when I was on my way to Canton he called me and said it was a zoom meeting and for many weeks he was asking for my witness list and once I gave it to him he did nothing with it now I hired him he put in limited appearance but every time we meet with him at his office he sounded very confident had a hundred question filled up almost two pages to ask my brother who was never even on the stands but he never reached out to one of my witnesses and fact they all got spooked shortly after I gave him the witness list he was also representing somebody else that was going through the same exact thing the same building and I asked him if he was representing them his name was Richard Nickerson and he told me he was not I have the court paperwork that shows he was representing him but during the housing case on July 15th which was supposed to be a status hearing they went forward with the trial and by the time I got on zoom my attorney his screen was black I could not see him and the other side already testified but then he had me plead the fifth for every question and we never ever discussed that and then they use that as evidence on the family and probate Court and it just happened to come out on the 13th when they filed an ex parte motion on the 14th and use that information to make me look bad my lawyer refused to appeal it so he said he wouldn't appeal it I must appeal it I believe you did that because if he appealed it he would have to answer to some of the things that he did the only thing he did was file a motion for an appearance and objection never filed an affidavit of objection now his picture is on the wall at Norfolk family and probate not that that means anything but he told me he was a gal and he was he looked like a great attorney and he acted like that in the office but why wouldn't he file an objection in an affidavit because it literally allowed everything to stay on record he didn't object to anything and then he quit right after both cases but I didn't allow him to back out of the housing case now he just sent me the Case files and he has a affidavit objection that I never signed he never ever mention that to me because I would have signed it he did try to have me sign a blank piece of paper without telling me what it was for but my other concern is that I was there in the courtroom on September 3rd objecting to my mother's guardianship when I was her proxy and the judge said that she never seen my proxy it was never passed in she doesn't know about it when it's in the medical report in Nicole Tara's affidavit who they based their whole case on who I never got to question now she constantly cut me off would not let me speak even my mother's attorney told me that she doesn't like me after the court hearing but I showed him a video of my visit with my mother and he didn't believe that was her he thought that was an old video and it wasn't it was a couple days previously that I was visiting her now with that information he you should have tried to do something with that information but he did nothing now when does an attorney back out right before the trial just because of a court date that she can't make her attorney didn't make the October appearance but we didn't get a new lawyer it made no sense but how can they be my mother's attorney and not file one motion or even come to see her at her home she told everybody that she did not want to go in a nursing home the gal that came by the house I had it on video Nicole terrace affidavit who we submitted emotion pointing out her lies of AMA discharge and showed the actual discharge paperwork and the judge denied it now she denied almost every single thing I tried to submit they filed an ex parte motion with my mother in the hospital safely and they said I was up there trying to remove her from the hospital yes I was there I wanted to take my mother home but there was no confrontation I just gave my mother clean clothes they just made allegations and that's all they needed now I have a audio recording of my interaction on the 13th I also have Nicole Tara who removed my mother from her home on August 12th and I have the 911 audio with the property manager stating that he called South Shore Elder services and the whole incident lasted about 15 minutes and I was downstairs with the EMS in the colterra for at least 10 so why did she lie about my mother wandering for 2 hours and the day before the police officers who supposedly found my mother and they won't give me the audio recording for that day cuz I don't believe she ever went anywhere but Nicole Sarah had my mother surrounded in the hallway and when I asked to take her back upstairs she refused to give her back to me I was hurt healthcare proxy and when I went to get her up to the hospital I was her invoked health Care proxy they had no right to keep her from me unless it was a medical hold or a psychiatric cold and I heard a rumor that she had a UTI I highly doubt they tested her for a UTI within a couple of hours that she was there okay they had to get her and remove her from the property because they were working with the property and my brother and his wife and South Shore hospital and I have audio recordings from many different dates but I'll give you a few November 11th 2023 November 20th 2023 that one's the most damning because it shows my brother's wife saying that she is calling on the behest of South Shore Elder services and that she's been speaking to the property management company why is she speaking on the phone first of all she has no rights she was the one going for guardianship and she incriminated everybody now also I have audio recordings from March 7th 2024 June 11th 2024 June 17th 2024 this one is Bridget from South Shore VNA saying that we are in the process of removing him as healthcare proxy and I also have several more from South Shore Elder services but Nicole Tara every time she called in a wellness check even the officer called her and asked why she called in this wellness check because my mother was with her secondary PCA and she was fine now I thought there was supposed to check on wellness checks not call them in how does she know my mother is in need of help my purpose and my points of this long email and I apologize is because I'm hoping I can get some help and some justice for the way my mother was treated the way I was treated nothing but accusations with no evidence to back it up now that I have is a very very extensive list of evidence I have reports 911 audio recordings I have witnesses that will attest to how I treated my mother and South Shore Elder services is supposed to coordinate hours if I need hours and instead they kept me on the adult foster care program for an additional eight months after I switch programs to Tufts because I was denied any services in July of 2023 they said due to need and do cuz I'm on the AFC program but I told them I do not want to be on this program the point being is I was approved for PCA help from January 1st 2024 and I never received any PCA services until July of 2024 and then I had to backdate it but nobody came out no care manager nobody came out to help us with the paperwork we were just left in limbo they even sent an Uber to pick up my mom the service that what they were paying for that a dementia patient cannot take a ride in an Uber without a chaperone now they promised me all this when they were in my building and solicited me and my mother with subs soft drinks chips and shirts bags all sorts of stuff and I didn't think insurance companies could do that but the owner of my building owns part of gather health he's on the board of South Shore hospital he's also the property owner with South Shore Elder services resides their office and they have a contract with the building to take care of the elderly but where was the help for many years that I was involved with them I even have audio recordings from 2020 3 2022 them saying they know the situation with my mom but they waited till 2024 to do anything because they needed my brother to help them remove my mother from her building because my mother had a disability now the building cut out mold in my unit with a sawzall failed to contain it and I had to pay for the cleanup and my mother was deathly ill and they found a growth in her lung a few weeks later but I had took her back and forth to doctor's appointments and all this is about cuz I called about my mother being assaulted at South Shore hospital and when I went to pick her up she was naked missing her clothes and I had to carry her out of there but no sneakers none of her clothes and it wasn't the first time now I don't know if you want me to quote laws and precedence and I'm sure there's plenty of it I'm just giving you proud of my story but the judge judge dubin violated her code of ethics many times throughout this case they didn't try at least restrictive they didn't they even got a restraining order against me to keep me from seeing my mother who I was a caretaker for 10 years now I'm going to send a demand letter I'm going to go to Superior Court wherever I have to go but I'm trying every option because this was a travesty my mother didn't deserve what happened to her they medicated her with black box Black label medications that will kill dementia patients which obviously I didn't know until she passed away like a lot of things I didn't know but I'm asking you to find it in your heart and then the law because she was discriminated against I had doctors notes I waited several years to be moved into a two bedroom and once they had to move me into a two bedroom they took us to court until the judge made them give us a two bedroom but they put us in one above the entry two floors higher when they knew my mother threw things out the windows because of her disability they were hoping that she would drop something on somebody but she was so good when we moved up into the new unit but yes once in awhile she did get a little confused it was a new unit but I waited almost 3 years for that two bedroom unit and it was almost exactly one year to the day when we made an agreement in 2023 that they had my mother out of the building now even her care management team and her insurance switched on August 13th 2024 and this plan was made up 6 months prior what are the odds of that that's impossible but there is much more like social services had an evaluation from a senior care specialist and she only suggested GPS bracelets and locks and Nicole Tara hit that information never showed the judge and my lawyer who was supposed to represent me and my interest how does he not put in an affidavit of objection that was step two and he couldn't even do that for me he let everything they say stand the case from Norfolk family and probate was and no 24 5129 GD but everything was sealed so I couldn't see it my lawyer wasn't showing me anything in all alone he's making me feel like he's fighting for me I have much more to talk about but I don't want to ramble too much
Other information	The medical certificate that they got back in May 30th 2024 and they started the petition June 4th and then they had a hearing on June 5th that we weren't there but they do say my mother was there no she was not and neither was I but when I came on the 11th Nicole Tara called in a wellness check hoping to catch me leaving my mother alone this woman should not be a PSA because she is not trying to protect people and trying to keep them in their home now Laurie Palmer who evaluated my mother on May 10th and even her primary care doctor who I brought to every other week for an evaluation and a checkup and the adult foster care program that I was on for almost 4 years send a nurse in a CNA by twice a month they never had a problem with my caretaking abilities and they did not accept any of this that I try to submit now they passed everything for them installed everything for us until it was too late now they made my brother I believe November 20th on December 3rd but I never got notified and the farm was literally submitted supposedly on this November 20th but I never got a notice I don't think they have any certificates of service for anything they serve me in my property manager was also breaking into my mail which I called 911 many times I pray you hear my plea and I do have much more evidence case law and emotions that I would like to submit but as I am not an attorney I am looking for some guidance thank you very much and look forward to hearing from you thank you James Sonia on behalf of My Mother Evelyn Egan and myself individually
